성모병원 관련 추적 60분을 보고...

먼저 백혈병 관련 환자와 그 가족분들에 심심한 위로의 말씀을 드리며 아픈 이들이 쾌차하시기를 바라며 이번 일로 받은 상처가 빨리 봉합되기를 또한 바랍니다.

대한민국의 의사라면 누구나  성모병원 같은 사태가 조만 간에 벌어질 줄 예상하고 있었다고 해도 지나친 표현이 아닙니다. 내심 이런 사태를 바라고 있었을 수도 있습니다.
성모병원에서는 왜 급여가 분명히 되는 약을 비 급여로 투약 하였으며  이 순간에도  우리나라의 모든 병원에서 비 급여 항목으로 처방이 나가고 있는 이유는 무엇일까요?
그 동안 주섬주섬 학습한 것들을 토대로  몇가지 문제를  적어보겠습니다.

1.평가기준의 비공개

물론, 대략적인 보험 인정기준이라는 책이 있고, 그 책에서 각 진료행위에 대한 인정기준 및 수가를 설명하고 있습니다. 하지만, 아주 세부적인 기준은 절대 비공개 입니다. 아니, 솔직히 말해서 그런 거 없다”라고 판단하고 있습니다. 하나의 예를 들면  지난 번글의  ‘처음”님의 예가  해당될 것입니다.물론  세부 기준을 전 잘 모릅니다.예를 드는 셈치고 보아주십시오.

관상동맥 조영술을 하면서 사용해야 하는  카테터(도관) 개수에 대한   알고 있는 기준이 1회에 하나인 경우라면  필요에 의해  추가로 더 검사한다거나, 카테터가 망가지거나 한 경우에 카테터를 추가로 청구하면 삭감이라는 것이고 병원에서는 손해 볼 수 없으니  환자에게 비 급여로 청구를 합니다.

문제는 이러한 경우에 기준이 될 수 있는 심평원 진료비 심사 지침이 비공개라는 겁니다. 병원 측에서 카테터 추가사용 분을  비급여로 환자에게 돌리게 된 이유는 전에 청구 하였을 때 심평원 측에서 삭감을 하였기 때문입니다. 그래서 심사기준이 이러이러할 것이라고 짐작해서 청구를 안 하는 것입니다.

그런데  관상동맥 조영술 검사 받은  받은 환자가 나중에 보험공단이나 심평원에 이의신청 하면 추가로 사용한 카테터 비용은 '급여해당'이라고 통지가 갑니다. 동시에 자동으로 병원으로 가야 할 보험료를 삭감하고 환자에게 대신 지급합니다. 이러면 일할 맛이 나겠습니까? 이런 이해할 수 없는 혼란을 막기 위해서는 심평원 심사기준을 투명하게 하여야 합니다.

사실, 아마 심사기준은 다음과 같이  되어 있을 것입니다.

<"관상동맥 조영술은  허혈성  심질환이 의심되는 경우에 1회에  카테터 하나의 사용을 인정하며   ‘의사의 판단에 따라 ‘추가적으로   카테터  사용도 허용 할 수 있음" >

병원에서  2개를 청구하면 앞의 1회라는 문구를 적용해서 삭감하고  환자가 민원을 넣으면 의사의 판단에 따라라는 문구를 적용시켜서 급여 해당이라고 답변하는 것입니다.그러니 추적 60분에서 처럼 찾아가서 어떻게  물어보아도  답변하기 좋게 되어있는 것입니다.

여러분은  적들이 몰려오는 치열한 전장에서  이십명 이상일 때만 수류탄을 터뜨리고 20미터 앞에서만 소총을 발사하고  탄알과 수류탄을  지정된  거리밖에서 추가로 사용한 경우   군사재판에 회부한다는 식의 지침이 있다면 전쟁 하겠습니까? 싸울 맛이 나겠습니까?

심사태만원이 심사기준 공개를 꺼리는 이유는 공개하면 '의사의 판단에 따라'라는 문구 때문에 꼼짝없이 급여 해 주어야 하기 때문입니다. 그러니 기준을 비공개로 하고 그 뒤에 숨어 마음껏 삭감하는 겁니다.

사실, 심평원의 급여 판단 기준은 어제 추적 60분을 통해서 보듯이  건보 재정입니다. 돈이 좀 있으면 삭감 덜하고 떨어지면  희안한 핑계를 대서 다 삭감시키고  삭감도 지역에 따라 차이가 나게하고  동일 약이나 시술도 어떨 때는 인정하고 어떨 때는 인정 안하거나  일정빈도를 초과하면  초과 분부터는 알아서 삭감하여오고 있는 실정입니다.
심태원은 환자와 의사를 농락하는 작태는 그만 보여주시고, 진정 환자를 위한다면 심사 기준을 학실히 공개해 주시길 바랍니다.

2. 임의 비 급여 불인정

위에서  예로든 카테터(도관)의 추가 사용을  환자에게 미리 말하여 그렇게 해달라고 했을 때 추가로  비급여로 비용을 받고 시술해도 불법입니다.급여에 해당하는 한 개만  사용해야 하며 환자가 스스로 원해서 부담해도 불법입니다.이런 웃기는 짜장면이 어디 있답니까?

여기서도 느끼신 바처럼 욕먹는 분도 있지만 우리나라 대부분의 의사는 환자의 고통을 조금이라도 덜어주기 위해 노력하고 그렇게 하도록 배우며  수련 때 학습됩니다..당연히 불법인 줄 알면서 비 급여 처방 합니다.
또한 대학병원등은  진료분야와  청구하는 원무과등의 일이 분담되어 있어서  스탭들은  청구 관련한 것을 잘 모를 뿐 아니라   대개 교과서적인 진료를 하는 경향이 강하여 보험이 적용이 안되어도 환자를 위해서 치료하는 경우가 많이 있습니다.

근데 이런 문제가 불거져  나중에 공단에서 실사 나오던지, 아니면 퇴원한 환자가 민원 넣으면 (상식적으로 낫기를 바라면서  치료한 사람에게 어찌 이럴 수 있나요? 이런 인간들 있겠나 했더니 최근 브릭 댓글을 보니 이해가 갑니다) 병원은 비급여액을 토해내야 하고(다시  재청구 하더군요) 최선을 다한 치료도 불신의 눈초리를 받습니다. 이럴 때 그 분들이 이사실을 알았다면  속상하고 아플 것이며  의욕이 떨어지며 치료에 소극적이 될 수도 있습니다.사실 큰 병원은 삭감문제도 원무과에서 처리하니 나중에 보고받는 정도이니 다행스럽기도 하지만  본격적인 이슈가 되면 문제가 심각해 질 수 있습니다.

결국, 임의 비급여 문제는 이제 의사의 손을 떠났다고 봅니다. 중요한 당사자인 환자와 보호자들의 판단에 맡겨서 할 일입니다.환자가 직접 공단에 청구하는 방법도 가능합니다. 정부와 의사가 예전 처럼  미봉책으로 처리하는 일을 되풀이해서는 안될 것입니다.

3.그들은 진정  현실을 외면하고만 있는가?

그럼 건당보상공단, 보복부 공무원들은 이런 거 모르고 애매한 소리하는 것은  절대 아닙니다. 오히려 그런 사람들이 본인이나 가족이 아파서 입원하면 큰소리 칩니다. 다 알아서 할 테니 급여 비 급여 신경 쓰지 말고 다 해 달라고 합니다. 이것이 우리나라의 개살구 같은  의료보험의 실상입니다.

추적 60분을 보니 보복부사무관이  심사 기준이라는 것이 임의로 만든 것이 아니라 의사들의 자문을 얻어만든 것이다. 의사들 자신들이 기준을 만들어 놓고 그 기준이 잘못되었다고 하는 것은 모순이지 않느냐?라고 이야기하더군요.

의사들의 의견은 보험재정지출 최소화라는 가장 큰 명제에  의해 철저히 무시당해온 실상은 쏙 감추면서 자신들 입장만 이야기 하는 것을 보고  인간들이 저런 종족이었구나 하는 생각이 들었습니다.

보복부 공무원은  비현실적인 수가에 대해 의사들이   의료비 원가 상승분을 수가에 현실적으로 적용해 인상해 달라고 요구하지 않았다. 절차를 밟아 의견을 제시하면 제도적으로 보완을 했을 텐데  의사들이 이제껏 그런 요구를 하지 않았기 때문에 과거의 기준을 그대로  적용하고 있다는 식으로 거짓말 하더군요.

왜 요구를 하지 않았겠습니까? 혀가 마르고 닳도록 주장했지만 역시 돈이 없다는 이유로 의견을  묵살했으면서 의사들이 전혀 요구하지 않았다는 식으로 구렁이 담넘듯 넘어가더군요. 그 뻔뻔한 얼굴 정말 역겨웠습니다.

이러니까 제가  이번 성모병원 사태를 종기가 곪을 데로 곪았다고  생각하고 환부를 도려내더라도  제대로 해보자고 하는  겁니다.

4.제가 주장하는 것은 간단합니다.

1)재정 부족으로  환자들 급여 제대로 못해주는 것을  솔직히 고백하고 병원이나 의사들 매도하지  말것이며   지출을 줄이기 위해서 임의삭감을 하지 말라는 것입니다.

2)건보재정관련 보험재정이 적자인 이유중 하나는 보건복지부나 일부 시민단체의 주장처럼  의료 공급자나 소비자의 잘못이 아니라 재정관리자인 건강보험공단과  심평원 그리고 감독 책임자인 보건복지부가 재정관리에 실패했기 때문입니다.
감사원 지적에 의하면 건보공단 조직의 방만한 운영으로 인한  재정낭비가 심각한 수준입니다. 재정 관련하여  빛좋은 개살구 정책만 남발하지 말고 공단을 축소하던  말던  효율적인 대책을 수립하기를 바랍니다.

2)비공개로 하여  병원에서 청구하면 삭감, 환자가 청구하면 민원 들어주는 그런 따위 짓 그만 하고  심사기준 을 투명하게 공개 하고  현실적인  논의를 통해서 진료비  심사제도를 개선하기를 바랍니다.

3)이번 과 같은 임의 비급여문제는  의사나  정부단체가 정할 것이 아니고  당사자인 환자들이  판단하면 제일 좋을 것입니다.
그래서 필요하면  정부에 정당한 요구를 하고 재정이 어려우면  일부를 부담하거나 하는 식으로 말입니다.
환자와 보호자들이 필요하지 않다고 판단해서  지정된 치료하다  손해보아도 할 수 없습니다. 만약에 그렇다면  우리 사회의 수준이 그 정도이니  의사입장에서  더 나은 상황,좋은 세상이 오기를   인내하면서  기다릴 뿐인 것입니다.
